First: do not stop Xanax cold turkey. Withdrawal can cause grand mal seizures and is one of the few withdrawals that can be fatal. Second: counterfeit Xanax pills laced with fentanyl are now a leading cause of overdose deaths. If you are buying Xanax off-prescription, assume fentanyl is in it.
Understanding Xanax Addiction
Xanax (alprazolam) is the most prescribed benzodiazepine in the United States, and one of the most dangerous to stop taking without medical help. Because Xanax is short-acting, withdrawal comes on fast and hits hard. Seizures, hallucinations, and life-threatening complications can occur if you stop abruptly. Counterfeit Xanax
If you are buying Xanax off-prescription, the pill in your hand is probably not what you think it is.
DEA seizures show that more than 70% of counterfeit prescription pills sold as "Xanax bars" now contain fentanyl, often in lethal doses. These pills look identical to pharmaceutical Xanax: same shape, same color, same imprint. Many overdose victims thought they were taking a Xanax bar to take the edge off, and instead got a fatal dose of fentanyl.
If you are using Xanax that did not come from a pharmacy, the safest thing you can do is call us. We can get you a clinical assessment, real medical care, and a path off the pills before luck runs out. If you witness an overdose, call 911 and use Narcan if available. Narcan reverses fentanyl, not Xanax, so it will not hurt to try in either situation.
Recognizing the Problem
Drowsiness, slurred speech, poor coordination, slowed breathing, memory gaps ("blackouts" while awake), confusion, dizziness. Withdrawal between doses brings rebound anxiety, sweating, tremor, racing heart, panic.
Running out of medication early, doctor shopping, taking it for reasons other than prescribed, taking higher doses, combining with alcohol or opioids (extremely dangerous), buying off-prescription, secrecy around pill bottles, mood changes that track the dosing schedule.
Memory problems, cognitive dulling, paradoxical worsening of anxiety, falls and accidents, declining work performance, conflict with partners or family, dependence that makes the original anxiety much worse on the way down. Counterfeit-pill overdose is a real risk for anyone buying off-prescription.
The Science
Xanax is short-acting, which means it wears off quickly, prompting more frequent dosing. Physical dependence can develop in as little as 2-4 weeks of regular use, even at prescribed doses.
Xanax withdrawal is medically dangerous. Seizures can occur within 24-72 hours of the last dose. Never attempt to stop Xanax without medical supervision. Our detox program uses a gradual tapering protocol.
Long-term Xanax use causes memory problems, slowed thinking, and difficulty with concentration. These effects may persist for months after stopping.
Stopping Xanax often causes anxiety worse than what it was prescribed to treat. This "rebound" effect drives continued use and makes quitting feel impossible without professional help.

Xanax treatment runs on two parallel paths: a medical taper that protects against the seizure risk and rebound anxiety, and behavioral and psychiatric work that treats the original anxiety or panic with safer, evidence-based approaches.
Because Xanax has such a short half-life, our medical team typically substitutes diazepam (Valium, long half-life) for the alprazolam, then tapers the diazepam slowly. This smooths out the inter-dose withdrawal and protects against seizures.
CBT-P has stronger long-term outcomes for panic disorder than benzos ever did. It teaches you that panic attacks are not dangerous, that they end on their own, and that you can ride them out without medication.
SSRIs (sertraline, escitalopram), SNRIs (venlafaxine), buspirone, hydroxyzine, beta-blockers. Effective for anxiety without the dependence and tolerance pattern of benzos.
For panic disorder, social anxiety, and PTSD, exposure-based therapies systematically reduce the fear response. The original problem that drove the prescription does not have to be the rest of your life.

Dual Diagnosis
Every Xanax prescription started with a real anxiety problem. The original condition is still there, often worsened by years of medication that does not fully treat it.
Long-term Xanax use is paradoxically associated with worse anxiety, worse panic, worse sleep, more depression, and cognitive problems. Recovery is not just coming off the medication, it is actually treating the original anxiety with approaches that give long-term relief instead of just symptom suppression.

Treat counterfeit Xanax exposure as a fentanyl risk. Carry Narcan. Make sure they are not using alone. Get them in for a clinical assessment as quickly as you can. The off-label market is now a different conversation than it was even five years ago.
Common Questions
Xanax has a short half-life (about 11 hours, much shorter than diazepam at 100+ hours). Short-acting benzos produce a more pronounced peak and crash, faster tolerance, more frequent inter-dose withdrawal, and more intense rebound anxiety. That clinical profile makes Xanax uniquely prone to dependence compared to longer-acting benzos. It is also the most-prescribed benzo, which means it is also the most-misused.
Yes. Xanax withdrawal is one of only a few withdrawals (along with alcohol and other benzos) that can cause grand mal seizures, status epilepticus, and death. This is why we never recommend stopping Xanax abruptly. Medical tapering, often with cross-titration to a longer-acting benzo like diazepam, protects against seizures.
Very serious. DEA seizures of counterfeit prescription pills now show that more than 70% contain fentanyl, often in lethal doses. If you are buying "Xanax bars" from anywhere except a pharmacy, the pill in your hand is more likely to be a fentanyl pill than a real Xanax pill. Many overdose deaths in young adults trace back to counterfeit Xanax exposure.
Acute withdrawal typically lasts 1-4 weeks. But many clients experience protracted withdrawal symptoms (rebound anxiety, sleep problems, cognitive issues, paresthesias) that can last 6-18 months. The Xanax page is unusual that way: the road back is genuinely long, and we are honest about that from day one.
Extremely dangerous. Xanax + alcohol or Xanax + opioids both depress breathing in additive ways. Most fatal overdoses involving Xanax involve at least one other depressant. If you are using Xanax in combination, please tell us during intake, because it changes the medical plan.
