What Happens During Alcohol Detox? 3 Things You Should Know

By Dr. Jason Powers, MD, Chief Medical Officer · April 18, 2026 · 4 min read

If you or someone you love is considering taking the first step toward sobriety, one of the most common questions is: what actually happens when you stop drinking? Understanding what happens during alcohol detox can take some of the fear out of the unknown that often stands between a person and the help they need.

What to Expect During Alcohol Detox

Here's an explanation from Dr. Jason Powers, Chief Medical Officer at Positive Recovery Centers:

Alcohol detox is a lot like the opposite of the effects of alcohol. While alcohol is mainly a depressant, a sedative, so you drink, you chill, relax, pass out, alcohol withdrawal is the exact opposite.

During withdrawal, patients experience heightened neurological activity, which can show up as seizures, an elevated heart rate and blood pressure, sweating, and tremors.

Why Alcohol Withdrawal Can Be Dangerous

Not everyone experiences the same severity. For some, symptoms stay manageable and brief, but for people with longer heavy-drinking histories, withdrawal can escalate into serious medical complications.

One severe risk is delirium tremens (DTs). As Dr. Powers explains, this condition, Latin for "crazy shakes," can involve seizures, loss of touch with reality, and psychosis. DTs are more likely for individuals with extended drinking histories, prior seizure episodes, or underlying health conditions.

Why Alcohol Detox Is Best Done Under Medical Supervision

Our medical detox program is built around patient safety and comfort. Depending on withdrawal severity, the medical team may use medications to reduce seizure risk, manage agitation, control elevated blood pressure, and address nutritional deficiencies that are common in long-term alcohol use.

After the acute detox window closes, the team talks through longer-term medication options that support sustained sobriety and reduce relapse risk. Detox is the first phase of recovery, not the whole thing.

Is alcohol detox the right starting point?

Our admissions team can tell you within one call whether medical detox is necessary or whether you can step straight into another level of care.

Alcohol Has Taken Enough. Let Us Help You Add Back What's Yours

Recovery offers restoration. Clarity, relationships, health, and joy return. The journey ahead has challenges, but the destination is worth the work. Most people step into residential treatment or an outpatient program after detox so the work continues with structure and support.
