The Research

Why 30 Days Isn't Enough

The 30-day rehab is mostly a product of insurance limits, and the research has never really supported it. For most people, 30 days gets you stabilized. The work of recovery is just beginning when traditional treatment ends.

The reason it matters: PAWS

Post-Acute Withdrawal Syndrome is what happens after the acute detox is over. Mood swings, sleep disruption, anxiety, low motivation, intrusive cravings. It is the brain rewiring itself through neuroplasticity, and it typically lasts 6 to 24 months, peaking in the first 1 to 3 months. For alcohol and opioids the worst of it resolves at 6 to 18 months. For benzodiazepines, longer. A 30-day program ends right when PAWS is starting to spike. Sending someone home into the worst of it, without structure, is how most relapses begin.

What Each Phase Actually Does

Recovery is not linear. Each of the four phases does specific work that the next phase builds on. Skipping ahead, or being discharged early, breaks the chain.

PHASE 1

Days 1-7

Acute care

Detox & Stabilization

Medical detox under 24/7 supervision. Withdrawal symptoms managed with appropriate medications, vitals monitored, sleep and nutrition restored. The goal is not just to clear the substance, but to land you in a stable enough place that the rest of treatment can actually do its work.

PHASE 2

Days 8-35

Residential

Residential Core

24/7 inpatient care in a community setting. Daily group therapy, individual counseling, psychiatric assessment, family programming, evidence-based modalities. This is where most "30-day rehab" programs end. For our long-term clients, it is where the foundation is laid for the work that follows.

PHASE 3

Days 36-90

Step-down

PHP + IOP Step-Down

Day-program structure with increasing autonomy. Move from PHP (20-30 hours per week) into IOP (3-4 days per week) as stability builds. PAWS symptoms peak here. Having structured treatment in place during that spike is what helps you ride the wave instead of getting pulled under by it.

Days 91-120

Reentry

Sober Living + Aftercare Onramp

Sober living at Montrose for structure and accountability. Job, family, daily life resume in a substance-free environment with peer support. Aftercare groups begin and continue for a full year past discharge at no additional cost. Support tapers gradually rather than stopping the day the program does.

After day 120, one full year of weekly aftercare groups and alumni community keeps you connected to the team that treated you. There is no single graduation day, just a gradual transition back into daily life.
